New Service Request

Welcome to the Jefferson County PWSD No. 12 Community! 

To start service with us, please stop by our office or print out the New Service Contract, fill it out and sign it, then mail it to us along with payment.  If you use this option, please call our office so we can make sure it arrives safely. The fillable form can also be sent via email to office@pwsd12.com and you can call to make payment.  We look forward to serving your water needs.

Please note that to start service, a signed contract and a deposit are required.  The deposit will be refunded when you discontinue service with us, less any fees owed.  Deposit amounts are noted on the form.

If you need a complete meterset installed, please contact our office for current pricing and lead-time. New metersets are installed as soon as is practical, but typically within one to two weeks, depending on our current workload.  By law, we must wait three business days for other utilities to be located once notified.  Prolonged inclement weather may delay the installation.  We ask that you plan ahead and not wait until water is needed to apply for a new service.

Typically, homes are supplied by a 3/4" service line and meter; however, larger homes and homes with lawn irrigation systems or large bathtubs will likely benefit from the additional capacity of a 1" service line and meter.  Ask your contractor or plumber if this might be right for you.

PWSD No. 12 employees will install all metersets 1" and under.  Metersets greater than 1" are installed by the customer's contractor to PWSD No. 12 specifications (see below for specifications and notes).
